What is Achalasia Cardia?
Achalasia Cardia is a disease of Esophageal motility and not a cardiac disease. The valve of the esophagus fails to relax in response to food or paradoxically it contracts. This leads to the inability to pass the food from the food pipe to the stomach. So patient feels multiple symptoms which include difficulty in swallowing, chest pain, and regurgitation of food.
How is it diagnosed?
If you have a long-standing issue as described above, you should consult your Gastroenterologist. He will advise you to undergo a UGI SCOPY. Typical findings on Endoscopy need confirmation on another test named Esophageal Manometry.
What is POEM?
As per the name, PEPR ORAL ENDOSCOPIC MAYOTOMY is a very specialized procedure done for the treatment of Achalasia Cardia. In the past surgery was done to cut the muscles located at LOWER ESOPHAGEAL SPHINCTER for treatment of Achalasia Cardia. Rather than performing surgery here, in this procedure, Esophageal muscles are cut through the Endoscopic route. A tunnel is made by special instruments beneath the mucosa of the Esophagus 5-7 cm above the level of the GE junction. Through this tunnel, once the muscle layer of the Lower sphincter is reached, muscles are cut through the micro-dissection technique (MAYOTOMY). POEM gives a long-standing and durable response and gives lifelong relief from this disease.
